Faculty_Staff Payroll Deduction form		Eligible Faculty and staff can opt to deduct membership fees directly from their UA paycheck.
Family Membership Form	Family Membership	Regents Policy P05.10.020.A. defines a dependent child as an unmarried natural or adopted child who is financially dependent upon the employee for support, and who is under 24 years of age. Parents, siblings, roommates, etc do not qualify as a dependent for src membership.
Statement of Financial Interdependent Partner	Financial Interdependent	A financially interdependent partner (typically a spouse) or a legal dependent. Please read and verify your eligibility listed in the form.
